INSTRUCTIONS

• Complete one request form for each non-consecutive date or event that you are requesting a speaker for, i.e., an event in separate weeks or months.

• Complete one form per request for each separate speaker that you are requesting, (i.e., Pastor Tyrone or Pastor Cynthia Marshall) to be scheduled at different times or on different days during the same event.

• Complete all required fields to ensure that the request is transmitted and processed in a timely manner.

• Please note "N/A" or "None" where required information is not applicable to your request.

• Note: Notification that your request has been received will be forwarded within 5 business days following the date of your request.
